Dear Patients,
It is with great disappointment that Dr. Foust must announce the closure of practice at Port Orange Gynecology. As a small private business, we have struggled against setbacks ever since the COVID lock-down. Five employees dwindled to one on Sep 29, 2021 with the unexpected early exit of our manager. We will no longer be able to provide your medical care. Existing appointments are cancelled, and we cannot reschedule them. You will have to find another Ob-Gyn office or other option for your routine or specialized gynecology care.
Luckily, we have excellent Ob-Gyn's in our medical community. If you need help choosing, it is best to contact your insurance company to find a provider in network with your insurance that is accepting new patients.
***As a reminder, your health records are confidential. The secure portal will remain open for now, but not beyond 2021. A copy of your records can be released to your new Ob-Gyn with your signed release form. You can use our form or theirs and either mail it or FAX it to 888.987.7157.
Remember, if you verified your PCP at each visit, then they should already receive visit summaries and copies of labs and imaging, including pap smears and mammogram reports.

CANCELLATION / NO SHOW POLICY:
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly "full" appointment book. If an appointment is not cancelled at least 24 hours in advance you will be charged a seventy five dollar ($75.00) fee; this will not be covered by your insurance company.
